Lahore: Irfan Saeed, Registrar of the Inquiry Tribunal handed over the report to Home Secretary Punjab today at Lahore High Court. Government of Punjab, Home Department vide notification No.SO (Judl.-III)9-84/2012 dated 30.01.2012 appointed Mr. Justice Ijaz ul Ahsan as Chairman of the Inquiry Tribunal with following TORs: to ascertain the cause(s) of death(s) and ailment(s); to determine if any such cause is relating to the drug(s) administered and procured from PIC, Lahore; if the drug reaction is established as the cause of death and or ailment, to determine the source, manufacturing, processing, storage, dispensing and dosage of such drug(s); to fix responsibility of lapses at each stage, to make recommendation for averting such like incidents in future.
During the proceedings, the Hon’ble Chairman Mr. Justice Ijaz ul Ahsan was pleased to co-opt Syed Shahid Nasir as an expert. During the proceedings, the Honourable Tribunal examined 72 witnesses including the public/affectees and also the local as well as the foreign experts on the subject. A delegation of WHO was also involved by the Tribunal to help in its investigation.The commission of the Honourable Tribunal accompanied the co-opted member of the Tribunal in its visit to the Efroze Chemicals at Karachi. Based upon the oral, documentary as well expert evidence, the report in hand was finalized, which comprises 297 pages and made recommendations to avert such tragedies in future.
